abdominal pain with incontinence of loss of bladder control

Chief complaints:
Patient complaints of abdominal pain(epigastric region ) since 1 yr 
C/o Incontinence of loss of bladder control since 7 days
C/oBurning of urine since 2 days
Pain relieved by taking medication but medicine was not continuously used
C/o numbness in legs since 7 days
C/o weakness and body pains since one month

History of presenting illness:
H/o trauma to right hand 6 months back 
No c/o cough and weight loss 
No c/o pedal edema 

Past history:
She had a surgery done in right hand 
C/o heavy breaths and weakness before 6 months 
Where she got admitted 

Negative history:
C/o HTN (low BP)
No h/o diabetes
No h/o cad
No h/o tb 
No h/o asthma

Systemic examination:
No tenderness
No palpable mass
Hernial orifices : normal
Liver : not palpable
Spleen: not palpable

Personal history:
Married
Occupation: labour work
Appetite: decreased
Mixed diet
Bowel: regular
Micturition: normal ( loss of bladder control with burning sensation)
No known allergies
No habits / addictions

Family history:
Not significant

Treatment history:
She took medication for abdominal pain and stopped abruptly

General examination:
Pallor: mild
No cyanosis
No lympadenopathy
No malnutrition
Mild dehydration
Weight :60kgs
No icterus
No clubbing of fingers
No pedal edema
Grbs: 90mg/dl
Bp: 90/60
